Can I get Prescription Request Forms that are pre-printed with my practice information?

Yes, you can create these yourself. Click here to access the Prescription Request Form,
then either:

  • Type in your practice information and save that version of the form on your computer
    for repeated use or
  • Type in your practice information and make multiple copies to have ready for your
    ARESTIN® Rx Access submissions

Approval Process and Communications

What is the CVS/Caremark Specialized Pharmacy?

They are the ARESTIN® Rx Access pharmacy partner. Their staff does the work of
checking your patients’ insurance coverage for ARESTIN®, contacting patients to
notify them of approval and to collect co-pays, and shipping the filled prescription to
your office. This service is not available through regular CVS retail pharmacies

I faxed in the form. What happens next?

  • You’ll receive a faxed confirmation that the specialized pharmacy (CVS/Caremark)
    has received your form
  • CVS/Caremark verifies patient’s insurance benefits and contacts your office,
    whether approved or denied, within 72 hours
  • If approved, CVS collects patient’s co-pay by phone and ships product to your office
  • If denied, CVS does not contact patient. Your office must notify patient and discuss
    other options

What should I tell my patient to expect?

  • They will receive a telephone call within 3 business days from CVS.com if their
    prescription is covered. CVS.com will collect their co-pay either by credit or debit card
  • If the patient’s plan does not cover ARESTIN®, your office will contact them directly
  • Either way, patient should keep next appointment to start treatment of
    periodontal infection
  • At end of visit, give patient take-home materials that review these details

When should patient and our office expect a call from CVS.com about coverage determination?

  • Within 3 business days

If I get notification that a claim was approved, can I call the patient myself?

  • Absolutely. You can share the good news that the patient was able to receive coverage
    for ARESTIN®. Plus, it’s a great opportunity to remind the patient to take care of their
    co-pay and to keep their next appointment

How soon can I expect delivery of the product once the patient has paid the co-pay?

  • The product is shipped according to the requested delivery date you provide on your
    faxed Prescription Request Form. Product will not be shipped until patient pays co-pay

What does CVS/Caremark do during the process?

  • CVS/Caremark is our specialized pharmacy partner
  • They process the ARESTIN® Rx Access form you submit and determine the patient’s
    prescription coverage
  • If approved, they coordinate with insurer and collect patient’s prescription co-pay
  • They ship ARESTIN® directly to your office according to the delivery date you specify
    on your faxed Prescription Request Form

Who from my office should act as the contact person?

  • The contact person should be someone who can easily take phone calls from the
    specialized pharmacy
  • An administrative staff member may work better than a hygienist

What does the contact person need to do?

  • Receives calls from the specialized pharmacy
  • Records and tracks all prescription requests made to the program, determinations,

Your Patient's Prescription

If my patient does not show up for their scheduled appointment, can I use the product
on another patient?

No, any product obtained through the ARESTIN® Rx Access program is strictly for use of
the insured patient. Product is shipped to your office as a convenience to your patient
and product must be held in custody for the patient for whom the prescription was
written

What do I do with the product if my patient does not return for their ARESTIN®
treatment?

If your patient does not return for treatment, the ARESTIN® should be returned to the specialized pharmacy for destruction; it cannot be used in other patients

Can I order additional product for a patient and use it at both their initial scaling
and root planing visit and their maintenance appointment?

No, the program is designed to provide product for only one procedure. One prescription covers only one periodontal procedure. You can submit two prescription request forms for each visit

ARESTIN® Rx Access process

 

ARESTIN® is indicated as an adjunct to scaling and root planing (SRP) procedures for reduction of pocket depth in patients with adult periodontitis. ARESTIN® may be used as part of a periodontal maintenance program, which includes good oral hygiene and SRP.

IMPORTANT SAFETY INFORMATION

ARESTIN® should not be used in any patient who has a known sensitivity to minocycline or tetracyclines. The use of drugs of the tetracycline class during tooth development may cause permanent discoloration of the teeth, and therefore should not be used in children or in pregnant or nursing women. Hypersensitivity reactions that included, but were not limited to, anaphylaxis, angioneurotic edema, urticaria, rash, swelling of the face and pruritus have been reported with the use of ARESTIN®. In clinical trials, the most frequently reported nondental treatment-emergent adverse events were headache, infection, flu syndrome and pain.
