What is the CVS/Caremark Specialized Pharmacy?
They are the ARESTIN® Rx Access pharmacy partner. Their staff does the work of
checking your patients’ insurance coverage for ARESTIN®, contacting patients to
notify them of approval and to collect co-pays, and shipping the filled prescription to
your office. This service is not available through regular CVS retail pharmacies

Your Patient's Prescription
If my patient does not show up for their scheduled appointment, can I use the product
on another patient?
No, any product obtained through the ARESTIN® Rx Access program is strictly for use of
the insured patient. Product is shipped to your office as a convenience to your patient
and product must be held in custody for the patient for whom the prescription was
written
What do I do with the product if my patient does not return for their ARESTIN®
treatment?
If your patient does not return for treatment, the ARESTIN® should be returned to the specialized pharmacy for destruction; it cannot be used in other patients
Can I order additional product for a patient and use it at both their initial scaling
and root planing visit and their maintenance appointment?
No, the program is designed to provide product for only one procedure. One prescription covers only one periodontal procedure. You can submit two prescription request forms for each visit
ARESTIN® is indicated as an adjunct to scaling and root planing (SRP) procedures for reduction of pocket depth in patients with adult periodontitis. ARESTIN® may be used as part of a periodontal maintenance program, which includes good oral hygiene and SRP.
IMPORTANT SAFETY INFORMATION
ARESTIN® should not be used in any patient who has a known sensitivity to minocycline or tetracyclines. The use of drugs of the tetracycline class during tooth development may cause permanent discoloration of the teeth, and therefore should not be used in children or in pregnant or nursing women. Hypersensitivity reactions that included, but were not limited to, anaphylaxis, angioneurotic edema, urticaria, rash, swelling of the face and pruritus have been reported with the use of ARESTIN®. In clinical trials, the most frequently reported nondental treatment-emergent adverse events were headache, infection, flu syndrome and pain.
